Description :
Need application for BlackBerry 4.2.1+. Icon on Blackberry desktop. Click to open form. Form has 3 radio buttons, 2 text input boxes, Reset and Submit buttons. Reset clears the form. Submit opens the browser, goes to a specified URL with the form data appended to the URL (i.e. http://www.website.com?radio1=XXX&radio2=XXX&radio3=XXX&searchterm1=YYYYY&searchterm2=YYYY ).
